Bronchial Rheoplasty Improves Chronic Bronchitis Symptoms

In the RheSolve trial, electrical pulse therapy sustained improvements in cough and sputum scores through 24 months, whereas mechanical mucus removal alone had waning effects. The RheSolve trial, conducted by Michael Lester, MD, a pulmonologist and assistant professor of medicine at Vanderbilt Health in Nashville, Tennessee, has found that patients with chronic bronchitis who received bronchial rheoplasty treatment with electrical pulses experienced durable symptom improvement over 24 months from baseline. The trial included 273 people with moderate-to-severe chronic broncitis from 37 centers across the US and Europe who were randomly assigned to two treatment arms in a 2:1 ratio. One group of 185 patients received bronchedial rheaoplasty with the RheOx catheter, a device that can mechanically remove distal airway mucus. The other 88 participants received a sham-like mechanical comparative procedure that was similar to bronchiooplasty but did not deliver the energy pulses. Despite significant improvement in one secondary endpoint between the active intervention and control groups, the trial was ultimately negative due to unexpectedly greater improvement in the control group for the primary endpoint. The primary endpoint of the COPD and CASA-Q was not significantly different between the two groups, with both groups exceeding the predetermined minimally clinically important difference (MCID) by three or four times.
